Hospice and Palliative Care of Contra Costa is an independent, not-for-profit, community-based health care provider. Specially trained staff members provide quality, compassionate health care to adults and children facing the advanced stages of any life-threatening illness such as cancer, birth defects and other congenital anomalies, Alzheimer's, AIDS, ALS, and end-stage heart and lung disease. Care is usually provided in the patient's home, skilled nursing facility, or residential care facility. Hospice services include ongoing emotional, bereavement and spiritual support to patients and those persons close to them.
